Financial:

I am happy to discuss my rates and/or insurance applicability over the phone or during our initial consultation. I accept HMSA, UHA, HMAA, Tricare, MDX Hawaii (Queen's Health Care Plan), and HMSA Quest. I am not a medicaid/medicare provider.   Many insurance plans require pre-authorization for testing and insurance does not cover the cost of testing/evaluation unless the patient is suspected of having a significant mental health condition (i.e., DSM-IV Axis I disorder) and the testing is relevant to and important for diagnosis and/or treatment planning.  Typical cost for “out-of pocket” evaluations is between $600 and $900, which includes diagnostic interviews, face-to-face testing, scoring and interpreting of tests, a written report, and a debriefing session during which time the report will be reviewed.  As costs vary, it would be best for us to discuss this with you in person.

Cancellation:

If you are unable to keep an appointment, I ask that you kindly provide me with at least 24 hours advance notice. I simply ask for this advance notice so that I may offer this appointment to another client.  A fee will be charged the second time a patient fails to show up for an appointment or cancels without sufficient notice.
